Indications

  • Prostate Cancer: Palliative treatment in advanced stages, often in combination with LHRH analogues.
  • Sexual Deviations in Men: To reduce sexual drive in cases of severe hypersexuality.
  • Hirsutism and Severe Acne in Women: Often used in women with androgen-dependent conditions when other treatments are ineffective (in lower doses and combined with estrogens).
  • Androgenization Symptoms: Such as alopecia, seborrhea, or polycystic ovary syndrome (PCOS) – off-label in some cases.
Mechanism of Action

Cyproterone acetate works by:

  • Blocking androgen receptors, inhibiting the action of male sex hormones (testosterone and DHT).
  • Suppressing gonadotropin secretion, leading to decreased production of testosterone.
  • Exhibiting progestogenic effects, contributing to anti-gonadotropic activity and decreased libido.
Dosage and Administration
  • Form: Oral tablets
  • Typical Dose:
  • Men (Prostate cancer or sexual deviations): 50–100 mg once to twice daily
  • Women: Usually 2 mg/day (in combination pills like Diane-35; not Androcur 50 mg — which is a high dose)
  • Administration: Swallow whole with water after meals
  • Duration: As directed by physician depending on indication (long-term for prostate cancer, short-term/intermittent for others)
Side Effects
  • Common:
    • Fatigue, depression
    • Weight gain
    • Hot flashes, sweating
    • Breast tenderness or gynecomastia in men
    • Reduced libido or erectile dysfunction
  • Serious (rare):
    • Liver toxicity (hepatitis, liver failure — monitor liver enzymes)
    • Thromboembolism (risk increases when combined with estrogen)
    • Benign or malignant liver tumors
    • Osteoporosis with long-term use
    • Anemia (rare but reported)
Contraindications
  • Liver disease or history of hepatic tumors
  • History of thromboembolic disorders
  • Severe chronic depression
  • Known hypersensitivity to cyproterone acetate

  • Hepatic Risk: Liver function should be monitored before and during treatment.
  • Thrombotic Risk: Use with caution in patients with cardiovascular risk factors or history of blood clots.
