Indications

  • Advanced Prostate Cancer: Palliative treatment, often in combination with LHRH analogues.
  • Hypersexual Disorders in Men: Treatment of sexual deviation in cases where testosterone suppression is required.
  • Androgen-Related Conditions in Women (lower doses): Such as severe acne, hirsutism, and androgenic alopecia (used in combination with estrogens — not with Androcur 50 mg alone).
Mechanism of Action

Cyproterone acetate works by:

  • Blocking androgen receptors in target tissues, preventing testosterone and DHT from stimulating cancer or other tissues.
  • Inhibiting gonadotropin secretion, leading to reduced testosterone production from the testes.
  • Exhibiting progestogenic effects, further suppressing androgens and libido.
Dosage and Administration
  • Form: Oral tablet
  • Strength: 50 mg
  • Typical Dosage:
    • Prostate Cancer: 100–300 mg/day in divided doses
    • Sexual Deviation in Men: 50–100 mg twice daily
  • Administration: Take after meals with water
  • Treatment Duration: Long-term or as directed by specialist
Side Effects
  • Common:
    • Fatigue, loss of libido
    • Gynecomastia (male breast enlargement)
    • Weight gain
    • Sweating, hot flashes
    • Depressed mood
  • Serious (rare):

    • Benign or malignant liver tumors

Contraindications
  • Active or history of liver disease or liver tumors
  • History of thromboembolic events (e.g., DVT, stroke)
  • Severe chronic depression
  • Hypersensitivity to cyproterone acetate or excipients
  • Meningioma (current or history)

  • Liver Function: Perform regular liver function tests; discontinue if liver injury is suspected.
  • Thrombosis Risk: Caution in patients with personal or family history of clotting disorders.
  • Mental Health: Monitor for depression and behavioral changes.
  • Bone Health: Long-term use may increase fracture risk due to reduced bone density.
  • Not indicated for women in this dosage form.

Drug Interactions:

  • CYP3A4 inducers/inhibitors (e.g., rifampicin, ketoconazole): May alter serum levels of cyproterone.
  • Statins: Increased risk of liver damage or muscle breakdown.
  • Combination with estrogen: Increases risk of thromboembolism.
