Indications

  • Relief of Menopausal Symptoms: Treatment of moderate to severe symptoms due to estrogen deficiency, such as hot flashes, night sweats, mood changes, vaginal dryness, and sleep disturbances.
  • Osteoporosis Prevention: In postmenopausal women at high risk, when other treatments are not suitable.
  • Hypoestrogenism: In women with ovarian failure or surgically induced menopause.
Mechanism of Action

Estramon delivers 17β-estradiol continuously through the skin into systemic circulation.

  • Maintains consistent blood levels of estrogen over 24 hours
  • Binds to estrogen receptors in various tissues (brain, bone, vagina, blood vessels)
  • Regulates thermoregulation, urogenital health, mood, and bone density
Dosage and Administration
  • Form: Transdermal patch
  • Strength: Delivers 50 µg of estradiol per 24 hours (each patch contains 4 mg estradiol total)
  • Package: 24 patches
  • Application:
    • Apply 1 patch twice per week (every 3–4 days)
    • Place on clean, dry, hairless skin (lower abdomen, buttocks)
    • Avoid breasts and irritated skin
    • Rotate application sites
  • Duration: Continuous or cyclic HRT as prescribed by a physician
Side Effects
  • Common:
    • Skin irritation at the patch site
    • Breast tenderness
    • Headache
    • Nausea
    • Irregular bleeding or spotting
  • Serious (rare):
    • Venous thromboembolism (VTE)
    • Stroke or myocardial infarction
    • Breast or endometrial cancer
    • Gallbladder disease
    • Liver enzyme elevations
Contraindications
  • Estrogen-sensitive cancers (e.g., breast, endometrial cancer)
  • Active or history of thromboembolic events (DVT, stroke, MI)
  • Unexplained vaginal bleeding
  • Severe liver dysfunction or liver tumors
  • Known hypersensitivity to estradiol or patch components

Information

Warnings and Precautions:

  • Use lowest effective dose for the shortest time
  • Endometrial protection: Add a progestogen in women with a uterus to reduce the risk of endometrial hyperplasia or cancer
  • VTE and cardiovascular risk: Increases with age and duration of therapy
  • Regular monitoring: Annual breast exams, mammograms, and gynecologic assessments
  • Not for use in pregnancy or breastfeeding

Drug Interactions:

  • CYP3A4 inducers (e.g., carbamazepine, phenytoin): May reduce estradiol levels
  • Thyroid hormones: Estrogen may alter thyroid-binding globulin levels
  • Anticoagulants and corticosteroids: Possible interaction – monitor dosing
