Indications

  • Hypothyroidism: Treatment of primary (thyroidal), secondary (pituitary), and tertiary (hypothalamic) hypothyroidism.
  • Goiter: Treatment or prevention of euthyroid goiter.
  • Thyroid Cancer: Suppressive therapy after surgery for differentiated thyroid cancer.
  • Thyroid Suppression Test: As part of diagnostic procedures.
Mechanism of Action

Levothyroxine is a synthetic form of the endogenous thyroid hormone thyroxine (T4). It works by:

  • Regulating metabolic rate of cells.
  • Stimulating oxygen consumption and basal metabolic rate.
  • Influencing protein synthesis and enzyme activity.
  • Supporting normal growth and development (especially important in infants and children)
Dosage and Administration
  • Form: Oral tablet
  • Strength: 75 micrograms (µg)
  • Frequency: Once daily, preferably in the morning on an empty stomach, 30–60 minutes before breakfast.
  • Individualized Dose: Based on age, body weight, cardiovascular status, and thyroid hormone levels (TSH and T4).
  • Monitoring: Regular blood tests (TSH, free T4) to adjust dose.

Drug Interactions:

  • Absorption reduced by: calcium supplements, iron, soy products, antacids (take these ≥4 hours apart).
  • Enhanced metabolism with: rifampin, carbamazepine, phenytoin
  • Potentiates effects of: oral anticoagulants (e.g., warfarin)
  • Interferes with: insulin and oral hypoglycemics (adjust dose as needed)
