Indications
- Hypercholesterolemia: Treatment of primary hyperlipidemia (high LDL cholesterol)
- Mixed Dyslipidemia: Reduces LDL, total cholesterol, and triglycerides; raises HDL
- Cardiovascular Risk Reduction:
- Decreases risk of heart attack, stroke, and revascularization procedures
- For patients with existing coronary heart disease or diabetes
- Familial Hypercholesterolemia: In homozygous and heterozygous forms (as adjunct to diet or other therapies)

Simvastatin works by:
- Inhibiting HMG-CoA reductase, the enzyme responsible for cholesterol synthesis in the liver
- Increasing hepatic LDL receptors, enhancing LDL clearance
- Reducing total cholesterol, LDL-C, triglycerides, and mildly increasing HDL-C
- Form: Film-coated oral tablet
- Strength: 40 mg
- Usual Adult Dose:
- Start at 10–20 mg once daily in the evening
- Moderate to high-intensity: 20–40 mg/day (max 80 mg only in select patients with no muscle issues on lower doses)
- Administration: Once daily in the evening, with or without food
- Common:
- Headache
- GI symptoms (constipation, abdominal pain, nausea)
- Muscle pain or cramps
- Elevated liver enzymes
- Serious (rare):
- Myopathy
- Rhabdomyolysis (severe muscle breakdown; rare but potentially life-threatening)
- Hepatotoxicity
- Allergic reactions (rash, pruritus)
- Active liver disease or unexplained persistent elevations in liver enzymes
- Pregnancy or breastfeeding
- Concomitant use with strong CYP3A4 inhibitors (e.g., itraconazole, erythromycin, HIV protease inhibitors)
- Known hypersensitivity to simvastatin or excipients
