Drug Class: Alpha-1 adrenergic receptor blocker

Indications
- Benign Prostatic Hyperplasia (BPH):
- Relief of lower urinary tract symptoms (LUTS) due to BPH
- Improves urine flow and reduces residual urine
- Not indicated for hypertension

- Selective α1-adrenergic receptor antagonist
- Relaxes smooth muscle in bladder neck and prostate
- → Decreases urethral resistance and improves urinary flow
- Extended-release (ER) formulation provides steady effect over 24 hours
- Dose: 10 mg once daily
- Administration:
- Take immediately after the same meal each day
- Swallow whole; do not crush or chew (to preserve extended-release)
- Onset of action: within hours; full benefit over several days to weeks
- Common:
- Dizziness
- Headache
- Fatigue
- Upper respiratory tract infection
- Abdominal pain
- Less Common:
- Orthostatic hypotension (especially at initiation)
- Syncope
- Retrograde ejaculation
- Serious (rare):
- QT prolongation (rare; caution in patients with risk factors)
- Liver enzyme elevation
- Angioedema (very rare)
- Moderate to severe hepatic impairment
- Known hypersensitivity to alfuzosin
- Concurrent use with potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
- History of postural hypotension

Warnings and Precautions
- Risk of orthostatic hypotension: caution when initiating or resuming after interruption
- QT prolongation: use with caution in patients with congenital QT syndrome or those on QT-prolonging drugs
- Floppy iris syndrome during cataract surgery: inform ophthalmologist if using
- Not approved for use in women
Drug Interactions
- CYP3A4 inhibitors (e.g., ketoconazole, itraconazole): may ↑ plasma levels
- Antihypertensives: additive hypotensive effects
- Nitrates, PDE-5 inhibitors (e.g., sildenafil): caution due to BP lowering
Pregnancy and Lactation
- Not indicated in women
- Category B (animal data shows no harm, but not studied in humans)
