Drug Class: Beta-1 selective β-blocker (cardioselective)

Indications

  • Hypertension (high blood pressure)
  • Angina pectoris (chronic stable)
  • Post-myocardial infarction (secondary prevention)
  • Cardiac arrhythmias (e.g., supraventricular tachycardia)
  • Migraine prophylaxis (off-label)
  • Anxiety-related tachycardia (off-label)
Mechanism of Action
  • Selectively blocks β1-adrenergic receptors in the heart
    • Slows heart rate, reduces cardiac output, and lowers blood pressure
    • Decreases myocardial oxygen demand
Dosage and Administration
  • Form: Oral tablet, 100-count package
  • Usual Adult Dose: 25–100 mg daily in 1 or 2 divided doses
  • Typical Starting Dose: 50 mg once daily
  • Adjust based on therapeutic response and heart rate
  • Take with or without food, ideally at the same time daily
Side Effects
  • Common:
    • Fatigue, dizziness
    • Bradycardia (slow heart rate)
    • Cold extremities
    • Depression or mood changes
    • Gastrointestinal upset
  • Less Common:
    • Sleep disturbances or vivid dreams
    • Shortness of breath in sensitive individuals
  • Serious (rare):
    • Heart block
    • Worsening of heart failure
    • Bronchospasm (especially in asthmatics)
    • Masking of hypoglycemia symptoms in diabetics
Contraindications
  • Sinus bradycardia
  • Second- or third-degree heart block (without pacemaker)
  • Overt cardiac failure or cardiogenic shock
  • Hypersensitivity to atenolol or other β-blockers

Information

Warnings and Precautions:

  • Use with caution in patients with
    • Asthma or COPD (due to risk of bronchospasm)
    • Diabetes (can mask hypoglycemia signs)
    • Peripheral vascular disease
  • Do not stop abruptly – taper gradually to avoid rebound hypertension or angina
  • Adjust dose in renal impairment (atenolol is renally excreted)

Drug Interactions

  • Other antihypertensives: Additive hypotensive effect
  • Calcium channel blockers (e.g., verapamil, diltiazem): Risk of bradycardia/AV block
  • Digoxin: Increased risk of bradycardia
  • Insulin/oral antidiabetics: May mask hypoglycemia

Pregnancy and Lactation

  • Pregnancy: Use only if clearly needed; may cause fetal growth restriction
  • Lactation: Excreted in breast milk; monitor infant for bradycardia
