Indications

  • Hypercholesterolemia (elevated LDL cholesterol)
  • Mixed dyslipidemia (elevated LDL and triglycerides, low HDL)
  • Prevention of cardiovascular disease (CVD) in patients at high risk (e.g., diabetes, prior heart attack or stroke)
  • Familial hypercholesterolemia (heterozygous and homozygous types)
  • Secondary prevention in patients with established atherosclerotic cardiovascular disease (ASCVD)
Mechanism of Action

Atorvastatin inhibits HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis:

  • Decreases hepatic cholesterol production
  • Upregulates LDL receptors, enhancing LDL clearance from the bloodstream
  • Results in lowered LDL cholesterol, modest increase in HDL, and reduced triglycerides
  • Reduces cardiovascular risk by slowing plaque formation and stabilizing existing plaques
Dosage and Administration
  • Starting dose: 10–20 mg once daily
  • Typical dose range: 10–80 mg once daily
  • Dose depends on LDL levels, cardiovascular risk profile, and response
  • Taken orally, with or without food, preferably at the same time each day
  • Regular monitoring of lipid levels recommended
Side Effects
  • Common:
    • Headache
    • Muscle pain (myalgia)
    • Nausea
    • Diarrhea
    • Joint pain
  • Less Common:
    • Elevated liver enzymes (ALT/AST)
    • Insomnia
    • Mild GI symptoms
  • Serious (rare):
    • Rhabdomyolysis (muscle breakdown with risk of kidney damage)
    • Liver injury
    • Hypersensitivity reactions (rash, angioedema)
    • New-onset diabetes
Contraindications
  • Active liver disease
  • Unexplained persistent elevations in hepatic transaminases
  • Pregnancy and breastfeeding
  • Known hypersensitivity to atorvastatin or any component of the product

Information

Warnings and Precautions:

  • Monitor liver function before and during therapy
  • Use with caution in patients with a history of liver disease or heavy alcohol use
  • Risk of myopathy increases with higher doses and when used with certain drugs (e.g., fibrates, cyclosporine, macrolide antibiotics)
  • Patients should report unexplained muscle pain, tenderness, or weakness

Drug Interactions:

  • CYP3A4 inhibitors (e.g., clarithromycin, ketoconazole, ritonavir): May increase atorvastatin levels
  • Grapefruit juice: Can significantly increase drug concentration—limit intake
  • Fibrates or niacin: Increased risk of muscle-related side effects
  • Warfarin: Atorvastatin may enhance the anticoagulant effect
