Indications

  • Gram-negative Bacterial Infections:
  • Treatment of moderate to severe infections caused by susceptible Gram-negative bacteria, including Pseudomonas aeruginosa.
  • Indicated for:
    • Urinary tract infections, lower respiratory tract infections, septicemia, skin and soft tissue infections, intra-abdominal infections, and gynecological infections.
  • Often used in patients with β-lactam allergies (e.g., penicillin allergy) — minimal cross-reactivity.
Mechanism of Action

Aztreonam:

  • Inhibits bacterial cell wall synthesis by binding to penicillin-binding protein 3 (PBP-3)
  • Causes cell lysis and death in susceptible bacteria
  • Active only against aerobic Gram-negative bacteria (no activity against Gram-positive or anaerobic organisms)
Dosage and Administration
  • Form: Intravenous (IV) vial – 1 g per vial
  • Typical Adult Dose: 1–2 g IV every 8–12 hours depending on infection severity and renal function
  • Maximum daily dose: usually 8 g
  • Adjust dose in renal impairment
  • Administered via slow IV injection or infusion over 30–60 minutes
Side Effects
  • Common:
    • Local injection site reactions (pain, phlebitis)
    • Nausea, vomiting, diarrhea
    • Rash
  • Serious:
    • Clostridioides difficile-associated diarrhea (CDAD)
    • Elevated liver enzymes
    • Rare hypersensitivity reactions (though less common than other β-lactams)
Contraindications
  • Known hypersensitivity to aztreonam or any of its components

Warnings and Precautions

  • Use with caution in patients with a history of β-lactam allergy, despite low cross-reactivity
  • Monitor renal function, especially in the elderly or those with pre-existing kidney issues
  • Superinfection: Prolonged use may lead to fungal or bacterial superinfections

Drug Interactions

  • No major interactions; can be used safely with other antibiotics
  • Synergistic with aminoglycosides against certain organisms — may be co-administered

Pregnancy & Lactation

  • Pregnancy: Category B — appears safe, but use only if clearly needed
  • Lactation: Excreted in breast milk in low concentrations — use with caution
