Drug Class: Antidiarrheal, antacid, salicylate derivative

Indications

  • Temporary relief of:
    • Diarrhea (including traveler’s diarrhea)
    • Upset stomach
    • Nausea
    • Heartburn
    • Indigestion
    • Excess gas or belching
  • H. pylori eradication (part of combination therapy, off-label)
Mechanism of Action
  • Coats the stomach lining and protects against acid irritation
  • Reduces inflammation in the gut through salicylate action
  • Bismuth has antimicrobial activity against some gastrointestinal pathogens
  • Decreases fluid secretion into the bowel, slowing stool frequency
Dosage and Administration
  • Form: Oral tablet
  • Strength: 262 mg
  • Adults and children ≥12 years: 524 mg (2 tablets) every 30–60 minutes as needed
  • Max: 8 doses (16 tablets) in 24 hours
  • Children <12 years: Use only under medical advice
  • Take with or without food; drink fluids to prevent dehydration
Side Effects
  • Common:
    • Blackened tongue or stool (harmless, due to bismuth)
    • Constipation
    • Nausea
  • Rare but Serious:
    • Salicylate toxicity (especially if overdosed or used with aspirin)
    • Tinnitus (ringing in ears)
    • Allergic reaction (rash, itching, swelling)
Contraindications
  • Allergy to salicylates (e.g., aspirin)
  • Children or teenagers recovering from viral infections (risk of Reye’s syndrome)
  • Bleeding disorders or active gastrointestinal bleeding
  • Severe renal impairment
  • Concurrent use of anticoagulants (e.g., warfarin) — caution advised

Information

Warnings and Precautions:

  • Do not use for more than 2 days without medical supervision
  • Discontinue use if ringing in ears or hearing changes occur
  • May interfere with x-rays or stool occult blood tests
  • Pregnancy: Use with caution (especially during the third trimester)
  • Breastfeeding: Avoid due to salicylate transfer into milk

Drug Interactions

  • Aspirin or NSAIDs: ↑ risk of salicylate toxicity or bleeding
  • Anticoagulants (e.g., warfarin): May increase bleeding risk
  • Tetracyclines and quinolones: Bismuth may interfere with absorption
  • Methotrexate: Salicylate may enhance toxicity
