Indications
- Hyperphosphatemia in Chronic Kidney Disease (CKD):
- Used to control high blood phosphate levels in patients with end-stage renal disease (ESRD) on dialysis. Helps prevent bone disorders and vascular calcification caused by phosphate retention.

Calcium acetate:
- Binds to dietary phosphate in the gastrointestinal tract.
- Forms insoluble calcium phosphate complexes that are excreted in feces.
- Reduces phosphate absorption and lowers serum phosphate levels.
- Form: Oral capsules or tablets, 667 mg per unit (equivalent to ~169 mg elemental calcium)
- Typical Starting Dose: 2 capsules/tablets with each meal (total of ~6/day)
- Titration: Adjust based on serum phosphate levels; monitor regularly
- Maximum Dose: Determined by calcium and phosphate control
- Must be taken with meals to be effective
- Common:
- Hypercalcemia (especially when combined with other calcium sources or vitamin D)
- Constipation
- Nausea, vomiting
- Loss of appetite
- Serious (Rare):
- Vascular or soft tissue calcification
- Confusion or mental changes (in case of severe hypercalcemia)
- Arrhythmias
- Hypercalcemia
- Severe hyperparathyroidism not adequately controlled

Drug Interactions
- May interfere with absorption of:
– Tetracycline antibiotics
– Fluoroquinolones
– Levothyroxine
(Separate administration by at least 2 hours)
Pregnancy & Lactation
- Pregnancy: Generally considered safe at normal doses; use under medical supervision
- Lactation: Small amounts may pass into breast milk; considered compatible
