Combination ARB and diuretic for hypertension

Description

Generic Name: Candesartan Cilexetil / Hydrochlorothiazide

Drug Class: 

  • Candesartan – Angiotensin II Receptor Blocker (ARB)
  • Hydrochlorothiazide – Thiazide Diuretic


This is a prescription medicine, which requires a valid prescription!

Indications

  • Treatment of essential hypertension in adults
  • Used when monotherapy with candesartan or hydrochlorothiazide is insufficient
Mechanism of Action
  • Candesartan blocks angiotensin II type 1 receptors → vasodilation, ↓aldosterone, ↓BP
  • Hydrochlorothiazide inhibits sodium reabsorption in the distal tubule → diuresis, ↓plasma volume
    • Combined effect: potent blood pressure reduction through complementary mechanisms
Dosage and Administration
  • Usual dose: 1 tablet (16 mg / 12.5 mg) once daily
  • May be titrated based on BP response and tolerance
  • Maximum dose: 32 mg candesartan / 25 mg hydrochlorothiazide per day
  • Take at the same time each day, preferably in the morning
Side Effects
  • Common:
    • Dizziness or lightheadedness
    • Headache
    • Fatigue
    • Increased urination (especially early in treatment)
    • Metabolic Effects:
    • Hypokalemia (due to HCTZ)
    • Hyperuricemia (can precipitate gout)
    • Hyperglycemia
    • Slight increases in cholesterol or triglycerides
  • Serious:
    • Hypotension (especially with volume depletion)
    • Electrolyte imbalances (hyponatremia, hypokalemia, hypomagnesemia)
    • Kidney dysfunction
    • Angioedema (rare)
Contraindications
  • Hypersensitivity to candesartan, HCTZ, or sulfonamide-derived drugs
  • Pregnancy (teratogenic – ARB component)
  • Severe renal or hepatic impairment
  • Refractory hypokalemia, hyponatremia, or hypercalcemia
  • Anuria

Information

Warnings and Precautions:

  • Monitor electrolytes, renal function, and blood pressure during therapy
  • Use caution in:
     • Elderly
     • Patients with diabetes or gout
     • Volume-depleted or salt-depleted patients
  • Avoid excessive sun exposure (HCTZ may cause photosensitivity)
  • May interact with other antihypertensives, NSAIDs, lithium, or antidiabetics

Drug Interactions

  • NSAIDs: May reduce antihypertensive effect, impair renal function
  • Lithium: Risk of toxicity
  • Potassium supplements or potassium-sparing diuretics: Risk of hyperkalemia
  • Other antihypertensives: Additive hypotensive effects

Pregnancy and Lactation

  • Contraindicated in pregnancy (especially second and third trimester)
  • Not recommended in breastfeeding
