Description

Generic Name: Dextromethorphan hydrobromide
Brand Name: Robitussin DM, Silomat, Tuscalman, others
Drug Class: Antitussive (cough suppressant), morphinan derivative (non-narcotic)


This is a prescription medicine, which requires a valid prescription!

Indications

  • Dry (non-productive) cough due to:
    • Cold or flu
    • Bronchitis
    • Pharyngitis or upper respiratory infections
    • Irritating cough without mucus production
  • May also be used in some chronic cough cases (off-label)
Mechanism of Action
  • Acts on the cough center in the medulla oblongata
  • Suppresses the cough reflex without inhibiting respiration at therapeutic doses
  • Structurally related to codeine but lacks opioid and addictive effects in recommended doses
  • Also acts as an NMDA receptor antagonist at high doses (associated with dissociation/euphoria in misuse)
Dosage and Administration
  • Form: Oral tablet
  • Strength: 30 mg
  • Adults and children ≥12 years: 15–30 mg every 4–6 hours as needed
  • Maximum: 120 mg per day
  • Not recommended for children <6 years (unless advised by a healthcare provider)
  • Take with water; with or without food
Side Effects
  • Common:
    • Drowsiness
    • Dizziness
    • Nausea or upset stomach
    • Mild sedation
  • Rare but Serious:
    • Confusion or hallucinations (especially in high doses or misuse)
    • Serotonin syndrome (if combined with serotonergic drugs like SSRIs, MAOIs)
    • Allergic reactions (rash, swelling, difficulty breathing)
Contraindications
  • Hypersensitivity to dextromethorphan or excipients
  • Concurrent use of monoamine oxidase inhibitors (MAOIs) or within 14 days after stopping an MAOI
  • Children under 6 years (for tablet form)

  • Do not exceed recommended dose → risk of CNS effects, misuse
  • Not for use with productive (wet) cough unless advised by a doctor
  • May impair mental or physical abilities — caution with driving or machinery
  • Pregnancy: Category C — use only if clearly needed
  • Breastfeeding: Caution advised; limited safety data available

Drug Interactions

  • MAO inhibitors: Severe serotonin toxicity or hypertensive crisis
  • SSRIs, SNRIs, TCAs: Risk of serotonin syndrome
  • Alcohol and sedatives: May increase drowsiness
  • CYP2D6 inhibitors (e.g., fluoxetine, paroxetine): May increase DM plasma levels
