Drug Class:
- Enalapril – ACE inhibitor (Angiotensin-Converting Enzyme inhibitor)
- Hydrochlorothiazide – Thiazide diuretic

Indications
- Hypertension (high blood pressure), when monotherapy is insufficient
- May reduce the risk of cardiovascular events associated with hypertension

- Enalapril:
- Inhibits ACE → reduces angiotensin II → vasodilation
- Lowers aldosterone → decreases sodium and water retention
- Hydrochlorothiazide:
- Inhibits sodium/chloride reabsorption in the distal renal tubule → diuretic effect
- Combined effect enhances BP control via complementary mechanisms
- Form: Oral tablet
- Typical Adult Dose: 1 tablet (20/12.5 mg) once daily
- Adjust dosage based on clinical response and blood pressure levels
- Take with or without food, ideally at the same time daily
- Not suitable for initial therapy in patients with volume depletion or renal impairment
- Common:
- Dizziness, headache
- Fatigue
- Increased urination
- Cough (from ACE inhibitor)
- Electrolyte imbalance (e.g., low potassium or sodium)
- Serious (rare):
- Angioedema
- Hypotension
- Renal dysfunction
- Hyperkalemia
- Hepatic impairment
- Severe skin reactions (e.g., Stevens-Johnson syndrome)
- History of angioedema related to ACE inhibitor use
- Anuria or severe renal impairment
- Hypersensitivity to sulfonamide-derived drugs (HCTZ)
- Pregnancy (especially in 2nd and 3rd trimester)
- Concomitant use with aliskiren in patients with diabetes or renal impairment

Warnings and Precautions
- Monitor electrolytes (especially potassium, sodium) and renal function regularly
- May increase serum uric acid (risk of gout)
- Use caution in patients with diabetes, liver disease, or autoimmune conditions
- Avoid use in pregnancy and switch to safer alternatives if planning pregnancy
Drug Interactions
- Potassium supplements or potassium-sparing diuretics → ↑ risk of hyperkalemia
- NSAIDs → may reduce antihypertensive effect and worsen renal function
- Lithium → ↑ lithium levels and toxicity risk
- Alcohol or other antihypertensives → additive hypotensive effects
Pregnancy and Lactation
- Pregnancy: Contraindicated (ACE inhibitors can cause fetal injury/death)
- Lactation: Enalapril may be present in breast milk; avoid if possible
