Drug Class: Proton Pump Inhibitor (PPI)

Indications

  • Gastroesophageal Reflux Disease (GERD)
  • Erosive esophagitis (healing and maintenance)
  • Zollinger–Ellison Syndrome
  • Helicobacter pylori eradication (in combination therapy)
  • Prevention of gastric ulcers in NSAID users
  • Acid-related dyspepsia
Mechanism of Action
  • Irreversibly inhibits H+/K+-ATPase enzyme in gastric parietal cells
  • Blocks final step of gastric acid secretion
  • Reduces both basal and stimulated acid output
  • Provides long-lasting acid suppression
Dosage and Administration
  • Form: Enteric-coated tablet, 40 mg
  • Usual Adult Dose:
    • GERD/Erosive esophagitis: 40 mg once daily for 4–8 weeks
    • Maintenance: 20–40 mg once daily
    • Zollinger–Ellison: 40 mg twice daily (adjust as needed)
  • Take at least 1 hour before meals
  • Do not crush or chew tablets (enteric coating)
Side Effects
  • Common:
    • Headache
    • Abdominal pain
    • Constipation or diarrhea
    • Nausea, flatulence
  • Less Common:
    • Dry mouth
    • Dizziness or drowsiness
    • Rash or pruritus
  • Serious (Rare):
    • Vitamin B12 deficiency with long-term use
    • Hypomagnesemia (may cause muscle cramps, arrhythmia)
    • Clostridioides difficile–associated diarrhea
    • Osteoporosis-related bone fractures (long-term use)
    • Interstitial nephritis (immune-mediated)
Contraindications
  • Known hypersensitivity to esomeprazole, other PPIs, or formulation components
  • Concomitant use with nelfinavir (due to reduced antiviral efficacy)

Warnings and Precautions

  • Use the lowest effective dose for the shortest duration
  • Monitor magnesium levels during prolonged therapy
  • May mask symptoms of gastric cancer — evaluate alarm symptoms (e.g. weight loss, bleeding)
  • Caution in hepatic impairment
  • Use in pregnancy only if clearly needed (generally considered low-risk)

Drug Interactions

  • ↓ Absorption of drugs requiring acidic pH (e.g., ketoconazole, atazanavir)
  • Clopidogrel: may reduce antiplatelet effect (use alternative PPI if necessary)
  • May ↑ levels of: diazepam, phenytoin, warfarin (monitor if applicable)
  • Methotrexate: high-dose therapy may interact → consider withholding PPI temporarily
