Indications

  • Menopausal symptoms (e.g., hot flashes, vaginal dryness) in postmenopausal women ≥ 6 months post-menopause .
  • Postmenopausal osteoporosis prevention in women at high fracture risk when non-estrogen options are unsuitable .
Mechanism of Action
  • Replaces declining estrogen levels to alleviate menopausal symptoms and helps maintain bone mineral density 
Dosage and Administration
  • Form: Oral film-coated tablet; continuous daily dosing.
  • Dose: 2 mg estradiol once daily.
  • Regimen: Taken continuously—no breaks—with or without food 
  • Duration: Use the lowest effective dose for the shortest duration necessary; reassess yearly 
Side Effects
  • Common:
  • Headache, abdominal pain, breast pain/tenderness, weight changes, edema 
  • Vaginal candidiasis, cystitis, mood changes, migraines may occur .
  • Serious (Rare):
  • Increased risk of breast, endometrial, and ovarian cancers
  • Venous thromboembolism (DVT, pulmonary embolism) and stroke
  • Liver disorders, gallbladder disease, pancreatitis, meningioma, hypertension
Contraindications
  • Known, suspected, or history of estrogen-sensitive malignancies
    • e.g., breast cancer or endometrial (uterine lining) cancer
  • Undiagnosed vaginal bleeding
    • Any unexplained genital bleeding must be investigated before starting HRT
  • Untreated endometrial hyperplasia
    • Estrogen alone can worsen this condition and increase the risk of cancer
  • Active or previous venous thromboembolism (VTE)
    • e.g., deep vein thrombosis (DVT) or pulmonary embolism (PE)
  • Known thrombophilic disorders
    • e.g., protein C, protein S, or antithrombin deficiency
  • Arterial thromboembolic disease (past or current)
    • e.g., heart attack, stroke
  • Severe liver disease
    • Until liver function returns to normal

Warnings and Precautions:

  • Endometrial Hyperplasia and Cancer:
    Estrogen-only therapy increases the risk of endometrial hyperplasia and carcinoma in women with an intact uterus. Progestogen should be added if the uterus is present.
  • Breast Cancer Risk:
    Studies show a slightly increased risk of breast cancer with prolonged estrogen use. Risk returns to baseline within a few years after stopping treatment.
  • Venous Thromboembolism (VTE):
    Estrogen therapy increases the risk of VTE. Use cautiously in women with known risk factors (e.g., obesity, personal or family history, prolonged immobilization).
  • Stroke and Myocardial Infarction:
    Slightly increased risk in older postmenopausal women, especially over age 60 or with predisposing conditions (e.g., hypertension, smoking, diabetes).
  • Liver Function:
    Estrogens may impair liver function. Discontinue if jaundice or other liver abnormalities occur.
  • Fluid Retention:
    May cause sodium and water retention; use with caution in women with conditions such as heart or kidney dysfunction.
  • Mood Changes or Depression:
    Monitor women with a history of depression or mental health disorders.
