Drug Class: Second-generation H1 histamine receptor antagonist

Indications
- Chronic idiopathic urticaria (hives, itching, rash)
- Seasonal allergic rhinitis (hay fever)
- Allergic conjunctivitis
- Skin allergies (off-label)

- Selectively blocks peripheral H1 histamine receptors
- Prevents the histamine-mediated allergic cascade
- Does not cross the blood–brain barrier → minimal sedation
- Rapid onset and long duration of action
- Form: Oral tablet
- Strength: 180 mg
- Adults & children ≥12 years: 180 mg once daily
- Take with water (not fruit juice, which may reduce absorption)
- Best taken before meals
- Common:
- Headache
- Drowsiness (mild and uncommon)
- Dizziness
- Nausea
- Rare:
- Fatigue
- Tachycardia or palpitations
- Hypersensitivity reactions (rash, angioedema, anaphylaxis)
- Hypersensitivity to fexofenadine or any component of the formulation
- Severe renal impairment (dose adjustment may be needed)
- Concomitant use with fruit juices (can reduce drug efficacy)

- Use with caution in elderly or those with renal impairment
- Pregnancy: Generally considered safe (Category B); consult doctor
- Breastfeeding: Passes into breast milk — use with medical advice
- Avoid antacids containing aluminum or magnesium within 2 hours (may interfere with absorption)
Drug Interactions
- Antacids: Reduce absorption of fexofenadine
- Ketoconazole, erythromycin: May increase fexofenadine levels (usually well tolerated)
- Fruit juices (e.g., grapefruit, orange, apple): May reduce absorption via OATP inhibition
