Indications
- Treatment of essential hypertension
- Treatment of diabetic nephropathy in patients with type 2 diabetes and hypertension
- May be used as monotherapy or in combination with other antihypertensive agents

- Blocks angiotensin II type 1 (AT1) receptors
- Prevents vasoconstriction and aldosterone secretion
- Results in vasodilation, decreased blood volume, and lower blood pressure
- Slows progression of kidney damage in diabetic nephropathy
- Form: Oral tablet
- Strength: 150 mg
- Typical Starting Dose: 150 mg once daily
- Maintenance Dose: 150–300 mg once daily depending on response
- Can be taken with or without food
- Dosage adjustment may be required in renal impairment
- Common:
- Dizziness, headache
- Fatigue
- Nausea
- Elevated potassium (hyperkalemia)
- Less Common / Rare:
- Hypotension (especially in volume-depleted patients)
- Kidney function decline
- Muscle cramps
- Rash or hypersensitivity reactions
- Hypersensitivity to irbesartan or any component of the formulation
- Pregnancy (especially 2nd and 3rd trimesters) – risk of fetal harm
- Severe hepatic impairment
- Concomitant use with aliskiren in patients with diabetes or renal impairment)

Warnings and Precautions
- Monitor kidney function and potassium levels regularly
- Caution in patients with renal artery stenosis or chronic kidney disease
- Not recommended during pregnancy or breastfeeding
- Volume- or salt-depleted patients may be at higher risk of hypotension
- May be less effective in Black patients when used as monotherapy
Drug Interactions
- Potassium-sparing diuretics, potassium supplements: risk of hyperkalemia
- NSAIDs: may reduce antihypertensive effect and impair renal function
- Lithium: may increase lithium levels and toxicity
- Diuretics: may potentiate hypotensive effects
