Indications
Hypertension:
- Treatment of high blood pressure to reduce the risk of cardiovascular events such as stroke and myocardial infarction.
Heart Failure:
- To improve survival and reduce complications after a heart attack.
Post-Myocardial Infarction:
- To improve survival and reduce complications after a heart attack.
Diabetic Nephropathy:
- To reduce progression of kidney disease in hypertensive patients with type 1 diabetes and proteinuria.

- Lisinopril inhibits the angiotensin-converting enzyme (ACE), preventing conversion of angiotensin I to angiotensin II.
- This leads to vasodilation, decreased aldosterone secretion, and reduced blood pressure.
- The reduction in angiotensin II decreases vasoconstriction and sodium and water retention, lowering blood volume and pressure.
- Form: Oral tablets, 10 mg
- Typical Starting Dose:
- Hypertension: 10 mg once daily; may adjust based on response.
- Heart failure or post-MI: dosing individualized, often starting lower.
- Hypertension: 10 mg once daily; may adjust based on response.
- Maintenance Dose:
- Usually 20–40 mg once daily, up to 80 mg daily in divided doses.

- Administration:
- Taken orally, with or without food, at the same time each day.
- Common (usually mild and gastrointestinal):
- Cough (dry, persistent)
- Dizziness or hypotension (especially after first dose)
- Headache
- Fatigue
- Hyperkalemia (high potassium)
- Elevated serum creatinine
- Serious (rare):
- Angioedema (potentially life-threatening swelling of face, lips, tongue, or airway)
- Kidney dysfunction or failure
- Severe hypotension
- Neutropenia or agranulocytosis (rare)
- History of angioedema related to previous ACE inhibitor therapy
- Bilateral renal artery stenosis or severe renal impairment
- Hypersensitivity to lisinopril or any ACE inhibitor
- Pregnancy: especially in the 2nd and 3rd trimesters (can cause fetal injury or death)
- Use with aliskiren in patients with diabetes or renal impairment (contraindicated)
Information
Warnings and Precautions:
- Monitor blood pressure, kidney function, and serum potassium regularly.
- Caution in patients with impaired renal function or volume depletion.
- Avoid NSAIDs where possible; can reduce antihypertensive effect and worsen renal function.
- Inform patients to report signs of angioedema immediately.
- Not recommended during pregnancy or breastfeeding.
Drug Interactions:
- Potassium supplements or potassium-sparing diuretics increase risk of hyperkalemia.
- NSAIDs may reduce antihypertensive effect and impair renal function.
- Lithium toxicity risk increases when combined with ACE inhibitors.
