Indications

  • Broad-spectrum treatment of serious bacterial infections including:
    • Complicated intra-abdominal infections
    • Bacterial meningitis
    • Complicated skin and soft tissue infections
    • Hospital-acquired pneumonia (including ventilator-associated pneumonia)
    • Sepsis, febrile neutropenia, and urinary tract infections
  • Active against many Gram-positive, Gram-negative, and anaerobic bacteria
Mechanism of Action

Meropenem:

  • Inhibits bacterial cell wall synthesis by binding to multiple penicillin-binding proteins (PBPs)
  • Resistant to most β-lactamases, including extended-spectrum β-lactamases (ESBLs)
  • Results in bacterial lysis and death
  • Rapidly bactericidal
Dosage and Administration
  • Form: Intravenous (IV) vial – 1 g
  • Typical Adult Dose:
    • 500 mg to 1 g every 8 hours IV
    • Higher doses (2 g every 8 hrs) may be used for severe infections (e.g., meningitis or multidrug-resistant organisms)
  • Infusion over 15–30 minutes
  • Dose adjustment required in renal impairment
Side Effects
  • Common:
    • Nausea, vomiting, diarrhea
    • Headache
    • Rash, injection site reactions
  • Serious:
    • Seizures (especially at high doses or in renal impairment)
    • Clostridioides difficile-associated diarrhea
    • Hypersensitivity reactions (including anaphylaxis)
Contraindications
  • Hypersensitivity to meropenem or other carbapenems
  • Severe hypersensitivity to other β-lactam antibiotics (cross-reactivity possible)

Warnings and Precautions

  • Central nervous system disorders: increased seizure risk in patients with CNS pathology or renal dysfunction
  • Hypersensitivity: use caution in patients with a history of penicillin or other β-lactam allergies
  • Monitor renal function during therapy

Drug Interactions

  • Valproic acid: meropenem significantly reduces valproic acid levels — avoid combination
  • Probenecid: may increase plasma levels of meropenem

Pregnancy & Lactation

  • Pregnancy: Category B — no known teratogenic effects in animals; use if clearly needed
  • Lactation: Present in small amounts in breast milk; caution advised
