Oral 5-HT3 Antiemetic

Description

Generic Name: Ondansetron
Brand Name: Zofran (and generics)
Drug Class: 5-HT3 (serotonin) receptor antagonist


This is a prescription medicine, which requires a valid prescription!

Indications

  • Prevention and treatment of nausea and vomiting associated with:
    • Chemotherapy (CINV)
    • Radiotherapy (RINV)
    • Postoperative recovery (PONV)
Mechanism of Action
  • Form: Oral tablets
  • Typical Oral Dosing:
    • CINV (moderately emetogenic): 8 mg twice daily
    • Highly emetogenic chemotherapy: 24 mg single dose (max)
    • PONV: 16 mg orally 1 hour before anesthesia
      Administration: With or without food; swallow whole
Dosage and Administration
  • Form: Intravenous solution (ampoule)
  • Chemotherapy-induced Nausea/Vomiting:
    • Dose: 250 mcg IV administered over 30 seconds approximately 30 minutes before chemotherapy
      Postoperative Nausea/Vomiting (PONV):
    • Dose: 75 mcg IV administered over 10 seconds prior to anesthesia induction
      Note: Do not repeat dosing within 7 days
Side Effects
  • Common:
    • Headache
    • Constipation
    • Fatigue
    • Dizziness
  • Serious (Rare):
    • QT prolongation and arrhythmias
    • Serotonin syndrome (especially with other serotonergic drugs)
    • Hypersensitivity reactions including anaphylaxis
Contraindications
  • Hypersensitivity to ondansetron or any 5-HT3 antagonist
  • Concomitant use with apomorphine (risk of profound hypotension and loss of consciousness)

Information

Warnings and Precautions

  • Monitor ECG in patients with cardiac risk factors
  • Avoid use with other QT-prolonging medications
  • Risk of serotonin syndrome with serotonergic drugs (SSRIs, SNRIs, MAOIs)

Drug Interactions

  • Apomorphine: contraindicated
  • CYP3A4, CYP1A2, and CYP2D6: minor metabolism pathway; clinically significant interactions are rare
  • Serotonergic agents: additive risk of serotonin syndrome

Pregnancy and Lactation

  • Pregnancy: Category B – generally considered safe if clearly needed
  • Lactation: Excreted in breast milk; caution advised
