Indications

  • Hormone Replacement Therapy (HRT):
    • Used in combination with estrogen in postmenopausal women with an intact uterus to prevent endometrial hyperplasia caused by estrogen-only therapy.
  • Menstrual Disorders:
    • Treatment of secondary amenorrhea or irregular menstrual cycles due to progesterone deficiency.
  • Assisted Reproductive Technology:
    • Support of the luteal phase in IVF or other fertility treatments.
  • Prevention of Preterm Birth:
    • Used in some cases to reduce risk of premature labor (off-label use varies by country).
  • Other Gynecological Conditions:
    • Endometriosis, dysfunctional uterine bleeding.
Mechanism of Action

Progesterone is a natural steroid hormone that:

  • Binds to progesterone receptors in the uterus and other tissues.
  • Prepares the endometrium for possible pregnancy by converting it from the proliferative to the secretory phase.
  • Counteracts the proliferative effects of estrogen on the endometrium, reducing the risk of hyperplasia and cancer.
  • Regulates menstrual cycles and supports pregnancy maintenance.
Dosage and Administration
  • Form: Soft gelatin capsules (Weichkapseln), 100 mg progesterone per capsule.
  • Typical Dosage:
    • For HRT: 100–200 mg daily, usually for 10–14 days per cycle in combination with estrogen therapy.
    • For luteal support in fertility treatments: varies, often 200 mg daily or as directed by specialist.
    • For menstrual disorders: individualized dosing.
  • Administration:
    • Taken orally, preferably at the same time each day.
    • Should be swallowed whole with water.
Side Effects
  • Common (usually mild and gastrointestinal):
    • Drowsiness, fatigue
    • Breast tenderness or pain
    • Headache
    • Dizziness
    • Mood changes or irritability
    • Nausea or gastrointestinal discomfort
  • Serious (rare):
    • Allergic reactions (rash, itching, swelling)

Contraindications
  • Known hypersensitivity to progesterone or any component of the formulation
  • Undiagnosed vaginal bleeding or abnormal uterine bleeding
  • Active thromboembolic disorders or history of thrombophlebitis

  • Use with caution in patients with history of thromboembolism or cardiovascular disease.
  • Monitor for signs of depression or mood disorders during therapy.
  • Not recommended during pregnancy unless specifically indicated.
  • Avoid abrupt discontinuation if used long term.

Drug Interactions:

  • May interact with enzyme inducers (e.g., rifampicin, carbamazepine) which can reduce progesterone levels.
  • Effects of progesterone may be altered by hormonal contraceptives or other steroid hormones.
