Indications

Simvastatin is used for:

  • Hypercholesterolemia (high total and LDL cholesterol)
  • Mixed dyslipidemia
  • Primary and secondary prevention of cardiovascular events in at-risk individuals
  • Reducing the risk of myocardial infarction, stroke, and revascularization procedures
Mechanism of Action
  • Inhibits HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis
  • Increases hepatic LDL receptor expression → lowers circulating LDL-C
  • Also moderately reduces triglycerides and increases HDL-C
Dosage and Administration
  • Form: Oral tablet, 20 mg
  • Usual adult dose:
    • Start: 10–20 mg once daily in the evening
    • Max dose: 40 mg daily (lower in high-risk interaction scenarios)
  • Evening dosing recommended (cholesterol synthesis peaks at night)
  • Dose adjustments may be required based on lipid levels, age, renal/hepatic function, and drug interactions
Side Effects
  • Common:
    • Headache
    • Abdominal pain, constipation
    • Nausea
    • Muscle pain or weakness (mild)
  • Less Common:
    • Elevated liver enzymes
    • Fatigue or insomnia
    • Skin rash
  • Serious (Rare):
    • Myopathy or rhabdomyolysis (risk increases with high dose or interacting drugs)
    • Liver injury (hepatotoxicity)
    • Hypersensitivity reactions (rare)
Contraindications
  • Active liver disease
  • Unexplained persistent elevations in liver transaminases
  • Pregnancy and breastfeeding (Category X – contraindicated)
  • Concomitant use with strong CYP3A4 inhibitors (e.g., clarithromycin, ketoconazole)
  • Known hypersensitivity to simvastatin or any statin

Warnings and Precautions

  • Monitor liver enzymes before starting and as clinically indicated
  • Use with caution in patients with renal impairment, hypothyroidism, or age >65
  • Report unexplained muscle pain, tenderness, or weakness (especially with malaise/fever)
  • Avoid large quantities of grapefruit juice (inhibits CYP3A4 → ↑ toxicity)
  • Stop therapy immediately if myopathy or liver damage suspected

Drug Interactions

  • CYP3A4 inhibitors (e.g., erythromycin, ketoconazole): ↑ simvastatin levels → ↑ myopathy risk
  • Gemfibrozil: ↑ risk of rhabdomyolysis — avoid combination
  • Other fibrates and niacin: Use with caution
  • Warfarin: May potentiate anticoagulant effect — monitor INR
  • Diltiazem, amiodarone, verapamil: Limit simvastatin dose to ≤10–20 mg/day
