Indications
- Herpes Zoster (shingles)
- Genital Herpes (initial and recurrent episodes)
- Herpes Labialis (cold sores)
- Suppression of recurrent genital herpes
- Prevention and treatment of cytomegalovirus (CMV) in transplant recipients (off-label in some regions)

- Valaciclovir is a prodrug of acyclovir
- Rapidly converted to acyclovir in the body
- Inhibits viral DNA synthesis by competing with deoxyguanosine triphosphate
- Effective primarily against HSV-1, HSV-2, VZV, and to a lesser extent, CMV and EBV
- Form: Oral tablet, 500 mg
- Typical Adult Dosing:
- Genital Herpes (initial): 500 mg twice daily for 7–10 days
- Genital Herpes (recurrent): 500 mg twice daily for 3 days
- Suppressive Therapy: 500 mg once daily
- Herpes Zoster: 1,000 mg three times daily for 7 days
- Cold Sores: 2,000 mg every 12 hours for 1 day
- Take with or without food
- Dose adjustment required in renal impairment
- Common:
- Headache
- Nausea
- Abdominal pain
- Dizziness
- Less Common:
- Rash
- Fatigue
- Elevated liver enzymes
- Serious (Rare):
- Thrombotic thrombocytopenic purpura (TTP) / Hemolytic uremic syndrome (HUS) – especially in immunocompromised patients
- Acute kidney injury (more likely with dehydration or high doses)
- Neurotoxicity (e.g., agitation, hallucinations, confusion) – rare and usually reversible, more common in elderly or renally impaired
- Hypersensitivity to valaciclovir, acyclovir, or any component of the formulation

Drug Interactions
- Nephrotoxic drugs (e.g., cyclosporine, aminoglycosides): increased risk of renal toxicity
- Probenecid: may increase plasma concentration of acyclovir
- Cimetidine: can reduce acyclovir clearance slightly
