Indications

  • Obesity Management:
    • Xenical is used for weight loss or weight maintenance in overweight or obese adults (BMI ≥ 30 kg/m²), or BMI ≥ 28 kg/m² with risk factors (e.g., hypertension, diabetes, dyslipidemia).
  • Prevention of Weight Regain:
    • Helps prevent weight regain after initial weight loss.
  • Note: Should be used in conjunction with a reduced-calorie, low-fat diet and exercise.
Mechanism of Action

Xenical works locally in the gastrointestinal tract:

  • Inhibits gastric and pancreatic lipases, the enzymes that break down dietary fat.
  • Prevents the absorption of about 30% of the fat consumed in a meal.
  • Undigested fat is excreted in the stool, leading to a caloric deficit and weight loss.

Important: It does not suppress appetite or affect the central nervous system.

Dosage and Administration
  • Form: Hard capsules (Hartkapseln), 120 mg
  • Recommended Dose:
    • 120 mg three times daily with each main meal containing fat (during or up to 1 hour after the meal).
    • If a meal is missed or contains no fat, skip the dose.
  • Administration: Swallow with water. Maintain a nutritionally balanced, mildly hypocaloric diet with about 30% of calories from fat.
Side Effects
  • Common (usually mild and gastrointestinal):
    • Oily or fatty stools (steatorrhea)
    • Urgent or increased need to defecate
    • Flatulence with discharge
    • Abdominal pain or discomfort
    • Diarrhea
    • Fat-soluble vitamin deficiencies (A, D, E, K) with long-term use
  • Serious (rare):
    • Severe liver injury (very rare; monitor for jaundice or dark urine)
    • Oxalate nephropathy (kidney stones) in predisposed patients
    • Allergic reactions (rash, itching, anaphylaxis)
Contraindications
  • Chronic malabsorption syndrome (e.g., inflammatory bowel disease)
  • Cholestasis (impaired bile flow)
  • Hypersensitivity to orlistat or any ingredient

Information

Warnings and Precautions:

  • Liver and Kidney Function: Monitor in patients with pre-existing liver/kidney conditions.
  • Malabsorption Syndromes: Contraindicated.
  • Cholestasis: Contraindicated in patients with bile flow disorders.
  • Pregnancy and Lactation: Not recommended; safety not established.
  • Vitamins: Consider supplementation with a multivitamin containing fat-soluble vitamins, taken at least 2 hours before or after Xenical.

Drug Interactions:

  • Cyclosporine: Reduced absorption; administer 3 hours apart and monitor levels.
  • Levothyroxine: May reduce thyroid hormone absorption; monitor TSH.
  • Warfarin and anticoagulants: Monitor INR closely.
  • Antiepileptics: Possible decreased absorption and seizure control; monitor levels.
